  1.                  CONSIGNMENT AGREEMENT
  2.  
  3. This agreement is made on     [date]    between [name of
  4. shop]    , herein referred to as "Seller", and
  5. [individual]
  6.  
  7. Whereas     [individual]   wishes to sell [description of
  8. item(s)]  by consigning said item(s) to     [name of shop]
  9. for sale, it is understood:
  10.  
  11. That Seller agrees to display item(s) in a prominent place
  12. in his establishment.
  13.  
  14. That Seller will make every attempt to obtain the best
  15. possible price for the consigned merchandise and will accept
  16. no less than  $           as purchase price.
  17.  
  18. That for his efforts Seller is entltled to retain  [percent]
  19. of the purchase price.
  20.  
  21. That should a sale be effectuated, Seller shall forward a
  22. check for the amount of the full purchase price less the
  23. aforementioned    [percent]     to Consigneee within 10 days
  24. of the receipt of same.
  25.  
  26. That Seller represents that he maintains insurance for theft
  27. and damage, and that the consigned merchandise will be covered
  28. by said insurance while it is in his possession.
  29.  
  30. That Consignee agrees to leave the merchandise with Seller
  31. for a minimum of     [time]

  33. That should the merchandise remain unsold at the end of the
  34. consignment period and an election be made by the Consignee
  35. to remove said merchandise, any costs incurred by the delivery
  36. of same to Consignee shall be borne by Consignee.
